PA Child Abuse Forms - Wednesday, January 14, 2009 New Parents or Broken Service: You must receive state Child Abuse History Clearance, forms CY-113 and SP4-164. These can be obtained online at Pennsylvania Child Abuse History Clearance Forms(see link to two forms at bottom of that page). The cost of each clearance is $10 (please note no cash or personal checks are accepted.) It can take 4 to 6 weeks to obtain these clearances. You must receive an Act 114, Federal (FBI) Criminal History Report. These can only be obtained online at Applicant Fingerprint Registration (see registration section of that page). Please note this is a longer process because a registration form must be completed online and then you must go to a fingerprinting center to complete the process. There is only one center available in Westmoreland County. This information is also available online. The cost of this clearance is $40 (please note no cash or personal checks are accepted.) It can take 8 weeks to obtain this clearance. Parents Who Already Have Clearances: If you have clearances already and have not broken service to the school district, you will be grandfathered into the program and do not need to obtain the Federal (FBI) Criminal History Report unless you so desire. If you wish to obtain this clearance please follow the steps above.
